To receive the gift card, you need provide your email address, but your answers will remain confidential and your email will not be used in any other way without your permission.
In addition, the first 1,000 students to refer at least five eligible friends who take the survey will receive an additional $10 gift card! So take the survey and then share it with your friends on campus using the unique link provided at the end of the survey!
